Enhanced Primary Care Mental Health
Each surgery has a dedicated Mental Health Practitioner who receives referrals mainly from health professionals such as GP’s, practice nurses or other primary and secondary health care workers.
They can help with any patients over the age of 17 years and 6 months and provide assessment, signposting, and brief interventions for patients within the following criteria
- Mental health problems too severe/complex for Steps to Wellbeing (S2WB) but not severe enough to meet clinical thresholds for secondary care services
- Serious Mental Illness patients living in the community setting but not supported by secondary services (including SMI on depo medication from CMHT but not actively supposed by care coordinator)
- Awaiting S2WB step 3 intervention where the problem is worsening and there are limited coping skills
- Recent therapy from S2WB but requires further support.
